Help Me Grow is not a stand-alone program, but rather a system model that works with cross sector collaboration in order to build efficient and effective early childhood systems. The goal is to mitigate the impact of adversity and support protective factors among families so that all children can grow, develop and thrive to their full potential.

Four cooperative and interdependent Core Components
characterize the Help Me Grow System Model:

A Centralized Access Point

For the Help Me Grow model, this takes the form of a call center and web-based services, supported by a Family Resource Navigator.

Child Health Care Provider Outreach

 When potential concerns are identified early on, they can be easier to address. Child Health Care Provider Outreach supports early detection and intervention efforts an connects medical providers to the grid of community resources which will best support families.

Family & Community Outreach

Builds parent and provider understanding of healthy child development, supportive services available to families in our communities, and how both are important to improving children’s outcomes.

Data Collection & Analysis

HMG Impact Indicators tell an important story about the reach and impact of HMG on communities, families, and children across the HMG network. HMG as a system is designed to reach families both within and outside of the centralized access point, child health providers, and community partners. The Impact Indicators that HMG affiliates collect and report to National on an annual basis describe the spread and scale of their efforts in reaching key stakeholders. Community data is critical to understanding where the gaps in services are and what providers might overlap critical resources. The HMG database provides community data for the first-time, and coordinates data collection from multiple sources within the 

Northeast Wisconsin (Help Me Grow NEW) region and can provide a realistic picture about how children are doing.

Components of the Help Me Grow System include:

Making the Connection

Help Me Grow Family Navigators provide intake triage, referral and connection to development services and community resources.

Building the Network

Help Me Grow NEW develops ongoing relationships with programs in the community and supports the maintenance of an updated resource directory.

Educating Providers

Educating professionals about the importance of developmental surveillance and screening while promoting access to developmental services through Help Me Grow is critical to community support.

Identifying Gaps and Barriers

Data is collected and analyzed to document and identify needs to more effectively connect families to community resources.

Providing opportunities for Developmental Screening

Developmental screening and monitoring is available by calling Help Me Grow line.
